Over the last year we have had an increase in number of patients failing to respond to invites for their chronic disease reviews. The practice will routinely send out two invites either by phone or by post. Most chronic diseases require some form of blood monitoring. Blood monitoring is often required to safely continue to prescribe medication. Attending for your blood tests and review actually help to reduce pressure on our team and will subsequently help to ensure your conditions are being managed appropriately.
