Band 5 and 6 Nurse Assessors sought to work in NHS Cheshire and Merseyside’s CHC team

NHS Cheshire and Merseyside’s Continuing Healthcare (CHC) Team is recruiting to a number of AFC Band 5 and 6 Nurse Assessor posts, with opportunities available within community-based teams across Sefton, Liverpool, St Helen’s and Knowsley.

We are looking for people with registered nurse qualifications, who are interested in hybrid working roles – with core hours of up to 37.5 hours per week, Monday to Friday.

Successful candidates will provide nursing assessment and review for adults and older people presenting with long-term care needs, and who may be eligible for continuing health care, funded nursing care and jointly funded packages of care.

Using a person-centred approach, Nurse Assessors will provide assurance around the quality of commissioned care packages and the reporting of safeguarding concerns, in accordance with agreed interagency reporting procedures.
